The Complex Care System Planning Table Hamilton (CCSPT) is a committee made up of various Hamilton multi-sector agencies that are making their expertise available support services in Hamilton who are serving individuals with complex mental health and/or addictions needs. If you are a healthcare or social service provider, please use one of the forms below to refer a case to the CCSPT.
