The event will include a brief information session followed by one-on-one meetings with a social worker, physician, or nurse. It’s a great opportunity for clients to connect with Hamilton Urban Core Community Health Centre’s health team, discuss any health concerns, and receive referrals or have follow-up appointments scheduled.
